On page 5, in Annex II, title: for: "Long-term declaration for products having preferential origin status", read: "Long-term supplier's declaration for products having preferential origin status". On pages 8 to 12, Annex V is replaced by the following: "ANNEX V INFORMATION CERTIFICATE INF 4 AND APPLICATION FOR AN INFORMATION CERTIFICATE INF 4 Printing instructions 1. The form on which the information certificate INF 4 is issued shall be printed on white paper not containing mechanical pulp, sized for writing and weighing between 40 and 65 grams per square metre. 2. The form shall measure 210 × 297 mm. 3. Printing of the forms is the responsibility of the Members States; forms shall bear a serial number by which it can be identified.
